Guidance on navigating a very complex healthcare system. Act as the primary liaison between patients, families and medical staff. Advocate for patient needs to ensure they receive the highest quality care.
Ongoing Support
Clear guidance before, during and after appointments to ensure nothing is missed. Weekly check-ins to address progress and/or new concerns. Your loved ones physical and emotional well-being will always be my top priority.
Patient Centered Care
Specialized care to improve patient outcomes and prevent hospitalizations. From decompensated heart failure to uncontrolled diabetes, I work in the home and with your specialists to keep your loved one out of the hospital.
Hospital & Crisis Management
Hospital stays can be an overwhelming and stressful experience for patients and families. Multiple providers may be involved and information can be difficult to track. I aim to be that medical “family member” everyone needs.
